Fox beneficiaries ages 4 to 18 can complete their sports/school physicals and update their immunizations July 27 from 8 a.m. to noon as Fox’s Red and White Team will hold the School Readiness Day.

Appointments for physicals are required; to schedule an appointment call 955-8888, option 2. Children needing shots may simply walk-in, however, those ages 4 to 6 must have had a physical within the past 12 months to receive their immunizations.

The physical appointments are short for school and sports. If patients need refills or want to discuss other chronic illnesses unrelated to approval to participate in sports, appointments for those things will be scheduled for a later date with their primary care manager. The primary focus of these appointments are for the school/sports physical; if things arise from that, they will also be scheduled for a later date.

Additional school/sports physical appointment slots will be included in the daily schedule from July 22-26 for those who cannot attend July 27.

Parents and guardians should bring their child’s sports physical paperwork, if available, with their information filled out, in addition to shot records or a copy of their Alabama Blue Card. For more information, call 955-8888, ext. 1148.
